All Students wishing to grade must:

1] Have completed New Wave Insurance/ Membership form online.

2] Must have all three New Wave Badges on their Uniform.

Note: although it is possible for students to double grade, i.e. advance two grades if they perform extremely well at their test, however, students always grade for their next immediate belt level and do not have the option to choose which grade they are applying for.

New Students who have not purchased Badges or completed Insurance forms will not be allowed to grade.

Students are required to wear their FULL Taekwondo/ Hapkido uniform in order to grade. T-shirts cannot be worn.

Uniforms should be clean and ironed

Lastly, the information on this form is used to produce a grading certificate. If the info is incorrect a replacement certificate will cost £5.

ADDITIONAL INFORMATION:

1] Both form and payment MUST be received in order for students to test for their belt.

2] If either the form or payment are received after the booking date, an additioanl late fee of £5 beocmes applicable.

3] ONLY the late fee will be accepted after the booking date. The late fee can be paid in cash or via bank transfer.  There is NO option to pay the late fee via the website.

4] Forms and payments cannot be processed on the day of the event.

5] The latest it is possible to submit either form or payment is MIDDAY of the day before the event, after which the booking form will be closed and payments will not be accepted.

Grading Date: THURSDAY 29TH OCTOBER

Venue: Buchan Hall, Buchan Road, Nunhead, SE15 3HQ

Time: Arrive 9:30 am for a 10:00 am start

BOOKING DATE: MONDAY 26TH OCTOBER

FEE: AS PER GRADE APPLYING FOR.

For example: The fee for the Yellow Belt test is £35.

Grading Acknowledgement

I understand that this event is conducted under New Wave Academy of Martial Arts (NWAC) rules. I accept that NWAC representatives cannot be held responsible for the loss of personal belongings or injuries sustained during the grading, subject to applicable law. I agree to accept the grading officials’ decisions.

Photography, Video & Publicity Consent

NWAC may take photographs and video footage during grading events for club promotion, including use on its website, newsletters, social media platforms (Instagram, Facebook and TikTok), and other promotional materials.

By signing this form, I give permission for NWAC to use, reproduce and publish photographs and videos of myself and/or my child taken during the grading for these purposes, without payment or compensation.

Social Media Etiquette & Tagging

To protect the safety, privacy and reputation of all participants, I agree to:

• Behave respectfully towards grading officials, instructors, volunteers, students and families, both in person and online.

• Not post abusive, offensive, discriminatory or defamatory content about NWAC or its members.

• Respect other participants’ privacy and not share photographs or videos of other students without appropriate permission.

• Not record or livestream the grading without permission from the Lead Instructor.

• Tag @newwavemartialarts when sharing relevant grading-related content, where appropriate, and share responsibly.

Tagging Consent: By signing this form, I consent to NWAC tagging members aged 13 and over on their own public social media accounts, where provided. Members under 13 may be tagged via a parent or guardian’s public account. If I do not wish myself or my child to be tagged, I will notify NWAC in writing.

NWAC reserves the right to request the removal of online content that may put participants at risk or damage the club’s reputation, and to take appropriate action where its Code of Conduct or safeguarding policies are breached.

Parent/Guardian Consent: If the student is under 18, this form must be completed and signed by a parent or guardian on their behalf.
