All Students wishing to grade must:

1] Have completed New Wave Insurance/ Membership form online.

2] Must have all three New Wave Badges on their Uniform.

Note: although it is possible for students to double grade, i.e. advance two grades if they perform extremely well at their test, however, students always grade for their next immediate belt level and do not have the option to choose which grade they are applying for.

New Students who have not purchased Badges or completed Insurance forms will not be allowed to grade.

Students are required to wear their FULL Taekwondo/ Hapkido uniform in order to grade. T-shirts cannot be worn.

Uniforms should be clean and ironed

Lastly, the information on this form is used to produce a grading certificate. If the info is incorrect a replacement certificate will cost £5.

ADDITIONAL INFORMATION:

1] Both form and payment MUST be received in order for students to test for their belt.

2] If either the form or payment are received after the booking date, an additioanl late fee of £5 beocmes applicable.

3] ONLY the late fee will be accepted after the booking date. The late fee can be paid in cash or via bank transfer.  There is NO option to pay the late fee via the website.

4] Forms and payments cannot be processed on the day of the event.

5] The latest it is possible to submit either form or payment is MIDDAY of the day before the event, after which the booking form will be closed and payments will not be accepted.

Grading Date: THURSDAY 02ND APRIL

Venue: Buchan Hall, Buchan Road, Nunhead, SE15 3HQ

Time: ARRIVE 9:30 AM FOR 10:00 AM START.

BOOKING DATE: FRIDAY 27TH MARCH.

FEE: AS PER GRADE APPLYING FOR.

For example: The fee for the Yellow Belt test is £30.

I understand that this event is run under New Wave rules. I accept that I cannot hold any representative of NWAC responsible for any loss of personal items or for any injuries that may be sustained during the grading.

I also accept, in full, the decision of the grading officials at this event.

Publicity Release- NWAC, take photographs and video footage of students on a regular basis. This material is used to promote/advertise the club on various media platforms including: newsletters, web pages, social media pages (Instagram, Facebook and Tiktok) and other promotional material and products. 

I understand that by signing this form I GRANT NWAC permission to use, reproduce, and distribute photographs and videos of myself and/or my child(ren) in material that may be used for promotion or otherwise of NWAC.  It is clearly understood that no royalty, fee or other compensation shall become payable to me now or in the future by reason of such use.

If student is under 18, this form must be signed by a parent or guardian.
